Tips for Aging New Yorkers on the Health Plan Marketplace

This year, Medicaid has new income guidelines; many folks may now qualify who weren’t eligible before. Medicaid beneficiaries receiving home and community-based care services for more than 120 days must enroll in a Managed Long Term Care health plan. VNSNY CHOICE, an affiliate of the not-for-profit Visiting Nurse Service of New York (the nation’s largest not-for-profit provider of home- and community-based health services), urges folks to do their research and contact one of its trained representatives for help answering any questions.

Here are few key elements from VNSNY CHOICE experts to consider when choosing a health plan:

Personalized Care with Registered Nurse Supervision

A correlation between personalized care and registered nurse supervision provides the quintessential coordination of medical care and personal assistance needed from any health plan. It’s essential to seek a plan that mutually provides for both. From scheduling essential appointments and assistance with household tasks, to providing round-the-clock nursing care at home, both are equally important.

Presence in the Community

Find a health plan that works congruently with your community to provide comprehensive care that best fits your needs. For example, does the provider have partnerships with hospitals to ensure smooth transition from hospital to home? Are there relationships with pharmacists should medication management questions arise? Are support organizations and resources known to the provider?

A Good Liaison with Your Physician

Open and close communication between your primary care physician and your plan’s healthcare team is crucial to streamline care and avoid complications. Are there plans that your physician recommends?
